Grafts Used in Revision Rhinoplasty

Grafts used in revision rhinoplasty may be evaluated to support the nasal structure and to shape the surgical plan according to the individual patient. The need for grafts is determined based on the patient’s current nasal structure, previous surgical history, cartilage support, skin thickness, and functional needs.

  • They may contribute to the evaluation of the nasal support structure
  • They may be included in planning for nasal tip and nasal dorsum support
  • They may be evaluated in areas with asymmetry or tissue deficiency
  • They may be used according to the structural support needs in revision surgery
  • Aesthetic and functional goals may be considered together during planning

Septal Cartilage

If the patient’s septal cartilage inside the nose is sufficient, it may be one of the graft sources considered first in revision rhinoplasty planning. However, previous use of septal cartilage in earlier surgeries or structural problems inside the nose may affect the suitability of this option.

Evaluation features:

  • Since it can be obtained from inside the nose, cartilage may not need to be taken from an additional area
  • Because it is the patient’s own tissue, it may be evaluated in terms of tissue compatibility
  • In some patients, it may be included in surgical planning for nasal tip and nasal dorsum support

When may it not be sufficient?

  • If a large portion was used in previous surgeries
  • If there is significant deviation, weakness, or structural problems in the septum
  • If stronger support is required in revision surgery

Ear Cartilage

Cartilage taken from the ear may be evaluated in some revision rhinoplasty plans due to its more flexible structure. It may be used especially in areas with limited support needs or in some corrections around the nasal wing and nasal tip.

Evaluation features:

  • It can be shaped in certain areas due to its flexible structure
  • It may be evaluated for support needs around the nasal wing and nasal tip
  • It may be included in surgical planning in some patients with limited tissue deficiency

Areas of use:

  • Evaluation of nasal tip support
  • Supporting small tissue deficiencies
  • Situations requiring support in the nasal wing area

Limitations:

  • It may not be sufficient on its own in cases requiring large-volume or strong support
  • Suitability should be determined according to the patient’s current nasal structure and the amount of support needed

Rib Cartilage

Rib cartilage is one of the graft sources that may be evaluated in some patients who require stronger structural support in revision rhinoplasty. It may be included in surgical planning especially in cases where the nasal framework is weakened, the patient has undergone multiple previous surgeries, or there is a significant need for support in the nasal tip and nasal dorsum.

Evaluation features:

  • It may be evaluated when a larger amount of cartilage is needed
  • It may be included in planning for nasal tip and nasal dorsum support
  • It may be used in some patients who require structural support in revision rhinoplasty

Does it require additional evaluation?

The use of rib cartilage requires a separate surgical evaluation because cartilage must be taken from an additional area. In this process, the patient’s general health condition, the amount of support needed, the scope of the surgery, and the healing process should be considered together. Possible pain, sensitivity, scarring, and healing time may vary from person to person.

Evaluation of Synthetic Materials

In some cases, non-biological materials may be considered in revision rhinoplasty. However, the use of such materials is not suitable for every patient and should be evaluated in detail in terms of possible risks, tissue compatibility, infection possibility, and the need for long-term follow-up.

In revision rhinoplasty, the use of the patient’s own tissue may often be considered among the primary options. The graft source to be used should be determined according to the patient’s nasal structure and surgical needs.

How Is Personalized Graft Selection Made?

In revision rhinoplasty, graft selection should be planned individually according to the patient’s current nasal structure and surgical needs. The following factors are evaluated together in this assessment:

  • The patient’s nasal bone and cartilage structure
  • Available cartilage reserve
  • Support needs in the nasal tip and nasal dorsum
  • The number of previous surgeries and their effects
  • Skin thickness and tissue healing characteristics
  • Breathing function and intranasal structures

The same graft source is not used for every patient. In revision rhinoplasty planning, the need for grafts should be determined after examination and physician evaluation.

This content is intended for general informational purposes only. It does not replace diagnosis, treatment planning, or personalized surgical evaluation. The use of grafts and graft selection in revision rhinoplasty should be evaluated by the physician according to the patient’s condition.
